Instructions for Attending Course and Taking Closed Exam
1. Complete and submit Application for Operator Certification Exams 30 days prior to exam date to be eligible to take closed exam. Submit to La Dept of Health & Hospitals, Certification Office, PO Box 4489, Baton Rouge, LA 70821. Application for Operator Certification Exams can be found:
a.http://new.dhh.louisiana.gov/index.cfm/newsroom/detail/1802
b. by calling LRWA Office to have the form faxed to you
c. DON’T send Exam Application to LRWA, send to DHH. LRWA only needs class registration form (below or bottom portion of the downloaded PDF above).
Registration for Water Level 3 & 4 is closed – we have meet our maximum capacity for the room.
