Note:  Beginning in 2017, Louisiana Department of Health states that pre-approval will be required for ALL certification exams, therefore pre-application must be submitted to their office at least 30 days or more prior to the exam date for ALL those desiring to test.  Walk-ons at exam sites without pre-approval will no longer be allowed to test.

Instructions for Attending Course and Taking Closed Exam
In order to be registered for both the closed exam AND the class, these instructions must be followed completely and accurately.

Complete and submit Application for Operator Certification Exams 30 days or more prior to exam date to be eligible to take exam.  Submit to La Dept of Health & Hospitals, Certification Office, PO Box 4489, Baton Rouge, LA 70821.  Application for Operator Certification Exams can be found

at http://new.dhh.louisiana.gov/index.cfm/newsroom/detail/1802

(DON’T send Exam Application to LRWA, send to DHH.  LRWA only needs class registration form).

Complete and submit registration below along with payment to register for training course (only).  Please select only one (1) class and indicate if you are attending class to take the exam or for certification credit hours only.
