1. MUST complete and submit Application for Operator Certification Exams by January 15, 2014 to be eligible to take closed exam.
Submit to La Dept of Health & Hospitals, Certification Office, P O Box 4489, Baton Rouge, LA 70821.
Application for Operator Certification Exams can be found:
a. at www.lrwa.org by clicking on the heading “Training and Events” and then clicking on “DHH Operator Certification Information”
b. by calling LRWA Office to have the form faxed to you
2. MUST download  and submit registration form along with payment by January 22, 2014 or go to www.lrwa.org to register for training course. If registering by mail, Submit to LRWA, Attn: 40 Hr Certification Training, P O Box 180, Kinder, LA 70648.
